      Stop in at any of our center locations to schedule with our front desk.

      Additionally, your healthcare provider’s office staff can assist you with scheduling your appointment as all diagnostic imaging procedures must be referred by your physician and require a signed referral.

      It is extremely important for you to keep your scheduled appointment. If an emergency prevents you from keeping it, please call our office prior to your scheduled appointment time, to cancel or reschedule. This simple step will prevent unnecessary delays for all of our patients.

    • What to bring

      1. Referral Form

      The form completed and signed by your ordering healthcare provider

      2. Photo ID and Insurance Cards

      You will need to bring a form of identification and current insurance card(s)

      3. Prior Studies

      Any previous, relevant imaging exams and reports performed outside of our network

      4. Payment

      Uninsured Patients: Payment is required at time services are rendered
      Insured Patients: At time of service, you will be required to pay any deductible, co-pay or coinsurance dictated by your insurance plan
